When you book an appointment through our platform for a medical marijuana evaluation with one of our partner physicians, we may ask you for sensitive personal information such as your social security number and driver’s license photo. The reason we collect this information is because it is required by the Florida Department of Health’s Office of Medical Marijuana Use (OMMU) to register patients in the state’s Medical Marijuana Use Registry. This registry is used to keep track of patients who have been certified to use medical marijuana in Florida.
The OMMU requires certain personal information to be collected from patients, including their full name, date of birth, social security number, and driver’s license number. The OMMU also requires a clear photo of the patient’s driver’s license to be uploaded to the registry.
