Why do we need certain information when booking an appointment?

When you book an appointment through our platform for a medical marijuana evaluation with one of our partner physicians, we may ask you for sensitive personal information such as your social security number and driver’s license photo. We understand that this information is sensitive and we take great care to protect it.

The reason we collect this information is because it is required by the Florida Department of Health’s Office of Medical Marijuana Use (OMMU) to register patients in the state’s Medical Marijuana Use Registry. This registry is used to keep track of patients who have been certified to use medical marijuana in Florida.

The OMMU requires certain personal information to be collected from patients, including their full name, date of birth, social security number, and driver’s license number. The OMMU also requires a clear photo of the patient’s driver’s license to be uploaded to the registry.

By collecting this information from you, we are able to streamline the process of registering you in the state’s Medical Marijuana Use Registry. This ensures that you are able to obtain your medical marijuana card quickly and easily, and that you can begin using medical marijuana to treat your medical condition as soon as possible.

We understand that you may have concerns about sharing your personal information, and we want to assure you that we take the security and privacy of your information very seriously. We use industry-standard security measures to protect your data, and we will never share your information with third parties without your consent.
